1. I understand that by enrolling in this service, I have read and agree to abide by Movement Five Policies and Procedures, found on the Movement Five website; https://www.movementfive.com.au
2. I agree to pay for all of the days and sessions that I am enrolled in and those that are not cancelled within the relevant time frame.
3. I authorise that Movement Five Services is able to invoice for all transportation time in a bus and/or car (if needed) to and from the programmed activities, under the compliance of the appropriate legislations, regulations and Transport Operations.
4. I acknowledge that in the event that I fail to pay the fees on time, Movement Five Services may be at liberty to terminate forthwith the provision of services. I understand that if my fees are not paid, Movement Five Services may request that I go on a payment plan which will include Direct Debit.
5. I acknowledge that Movement Five Services is required to disclose information to regulatory government departments and other government agencies. I understand that the Movement Five Services adheres to the Privacy Act 1988 and will ensure that information in enrolment record is not divulged to another person unless necessary for the purpose of care or regulatory requirements.
6. I understand that I must provide Movement Fives Services with all health related issues, action plans and all mandatory medical documents.
